New AKC Entry Form

The AKC entry form has been newly revised to require the exhibitor’s signature on the reverse side of the entry form. The previous AKC entry form required the signature on the front of the form. The bottom box of the new form is now the space for an agent’s name, instead of the exhibitor’s signature.

Please note that unsigned entry forms are deemed invalid and will be returned by the superintendent.
